Behavioral Health Case Management

Targeted Case Management Services are designed to assist an individual and/or family in accessing needed community resources. Case Management Services are provided as support services to other available agency programs by state-certified providers. BHCM-IIs refer, link, and advocate to ensure clients have daily life essentials to actively participate in treatment. Examples would include locating a new primary care doctor, finding additional resources for food, locating necessary school uniforms or school supplies, applying for utility assistance, or locating parenting resources for special needs.

On June 11th, our current electronic health system will transition to a new and advanced system to better serve you: Athena. Prior to the transition date, you will be sent a registration link to create a new patient account in Athena.
